1. Are we talking about a overhand baseball throw to the other end? This type of play will easily call itself...I'd whack as soon as it was evident what the player was doing.

2. I don't care if it's me or my partner that issues the second T in this situation. If I'm envisioning your original play correctly, then I'd say that this is something so egregious that I don't think it matters who gets the 2nd. That's not to say that there isn't good value in having a partner(s) get the 2nd T, but if something is so egregious, I think you have to take care of business no matter what.
