As johnnyg suggests, PU was making stuff up.
Under FED rules, each team is allowed three "defensive charged conferences" during a game.
Each time the manager requests time to have a private conference with his pitcher and does not remove the pitcher, one charged conference is used up. (If he makes a pitching change, the conference is not "charged".)
After the 3rd charged conference, any time the manager requests time to visit the pitcher, he MUST change pitchers.
If he hasn't used any charged conferences in the game, he could visit the same pitcher 3 times in the same inning and the pitcher would be allowed to continue.
NCAA uses a kind of "hybrid" of the OBR and FED rules. Two trips in the same inning to the same pitcher requires the pitcher's removal; after three visits during the game without the removal of the pitcher, each subsequent visit requires a pitcher change.
