I think this is like the old test for pornography -- you know it when you see it. It's hard to imagine how it would really come up. Perhaps if a team was losing with 3 minutes left, was PO'd, and started calling lot's of excessive TO's, one after another, and taking the T's. Or if A is trying to inbound after a T, and B keeps stepping over and deliberately hitting the ball so A can't inbound, leading to T after T . . .
