The purpose and intent of the rule is very important here. The rule makes it clear that this is an extraordinary remedy, and should only be used when there is a clear disruption. The ONLY time I would ever make such a call is when there is a concerted effort by multiple individuals. The best example is the Florida game in the late '80s when the fans threw down tennis balls late in the game because one of the Florida players had reportedly spit on a kid at a tennis court. Or something like that.

It's also a shame that you wish to center your thoughts on this around some NCAA game from the 80s, but if you wish to use college games for references you might wish to check out the recent Montana/Montana State game in which a T was called on the home team for a fan throwing a promotional mini basketball onto the court.
