Thanks for clarification on the game cards. We also have a game/rating card we give to the coaches during the pregame.
We don't get many back.
Playoff ratings are handled the same way as yours. At playoff time, the coaches in the playoff have an opportunity to provide a "Ding List" that would disqualify an official from working their games.
