1) If the thrower is ready and you're in the act of handing the ball to him, ignore the substitution request if the sub isn't at the table..
2) If you're not in the act of handing the ball to him, wait until the sub hits the X and then bring him in.
Of course, if a whistle is blown, you now have no reason by rule not to allow the substitution.
That's just about the right officiousness imo.
