Colorado:
$55 for varsity (5 crew)
$41 for sub-varsity (unfortunately both freshman and JV play 12 minute qrtrs unless we can talk them into 10 min..which doesn't happen very often). Our association works 5 man crew in sub varsity as well for training and allowing younger officials more games.
