1) Thanks, that's about what we did (although there was some discussion whether the cautioned player "may" be substituted or "must" be substituted, and I don't think there are any restrictions on the number of times a player may sub / be subbed for)
4) That didn't seem to be true, although the match was a blow-out, so maybe no one wanted to stop the clock.
