Now, I don't do LL, but in one of my house leagues leading off is not allowed (along with some other restrictive rules), and if a runner is leading off at the time of the pitch he is warned once, and then called out. Generally speaking in that league runners need to started heading back to their base when the pitcher is on the rubber. It is possible that is type of situation the OP is referring to.
