Scrapper, good questions. Here are my thoughts.

1) - TEW - doesn't hurt the team since it is a warning and you do not indicate whether there had been any previous delays by that team.

2) a - Call the time-out following the switch of courts and benches.
b - wait for them to finish changing sides.
c - ask if he still wants the TO as he goes by you.
d - not necessary to request it a second time.
e - no, it is not an improper request.
f - doesn't matter which coach makes the request.
