- live ball / running clock ("normal play")
- live ball / stopped clock (throw-in or FT)
- dead ball / running clock (after a basket)

Don't forget the jump ball in the live ball/stopped clock category. In fact, there are only three ways to get a dead ball live, and all three of them begin with a live ball and a stopped clock.
