It is a mistake.
Actually, The wording at the end of 8-3-5b "unless in conflict with other rules" is nothing more than a mistake (Long Story) and will be removed in the 2009 Rule Book. You can draw a line through it for 2008.
So,
In the event of a B foul during a successful try, the succeeeding spot is always:
The subsequent kickoff in regulation, or,
The start of overtime.
With all due respect to Theisey and his sample play, any foul during a successful try would be penalized, and, while it may look, smell and taste like a PSK foul it can not be since PSK does not apply during a try. Hence, since all fouls during a successful try shall be penalized you are left with the two options above.
My understanding is the NFHS will issue a list of errors of which this is one.
