From what I was told, in MLB the crew chief or original calling umpire will make the change call. It's up to the crew; that is, on some crews if the original umpire wishes to make the reversal or sustaining call, he'll do so. If he would prefer the crew chief does it, then the CC will. If I'm not mistaken, it's the same way in NCAA (at least that's what I heard from one of the NCAA's gurus).
