I am going to guess on a few of these.

a. 1998-1999?
b. 2003?
c. 1996 is when I was wearing them. The next year I think they took the "collared" part out of the Official's Manual.
d. 1986 (or the year after Villanova won the NC over Georgetown.

I am sure someone will have definitive information. This is just off the top of my head.
