Blow the whistle, make T signal and then report the foul to the table.
Oh, you mean how to administer it and to whom the foul gets charged?
This is charged to both directly to the player and indirectly to the Head Coach as all team members are bench personnel during intermissions. Thus the HC loses his coaching box.
