The starting defensive person (F8 whom DH batted for) and DH "share" the same spot in the batting lineup. Each is allowed reentry privilege.
When the DH took the field defensively, then F8 was removed from the game. DH remained in the game and entered defensively---he did not "re-enter" at that point. The DH position was lost at that point. F8 later "reentered" with DH being removed. DH later "reenterd" for his first time---legal.
F8, for whom the DH originally batted and with whom he shares the batting spot, must be removed and is no longer eligible for any play in that game.
