I think that if the ball was in the box when it hit the batter, I'd rule it foul regardless of whether a foot was in or out. (I'd agree with Terri1's call on his play where the ball bounced up off the plate and into the batter.) If the ball hit the foot out of the box, or if the batter's first step out of the box was onto the (fair) ball, then I'd call the batter out, even if the other foot was in.
