It is "legal" to attack a serve unless it goes over the net or is touched by an opponent. So, if you shank the attack and it runs down the net or is hit by a teamamte, it is legal. Now, the chances of that happening are nil, but it is there.
As far as subs go, forget 'who' is where, but positons on the floor.
If you think of the positions from server around to center back as 1,2,3 etc. You have to go back into that positon if you sub out. When the team rotates, that position will change. This is where your lineup card will come in handy, you will always know where that positon is on the floor.
