Substitutes

A question about the practical application of Rule 3-3-5 and case book 3.3.5B. A player goes down hard and grabs his ankle. It looks serious so I beckon the coach onto the court. After 30 to 45 seconds, the player gets up and "walks it off" and the coach walks back to the bench and the player stays on the court. At this point what's the procedure? Should I walk over to the coach and say, "If you want to leave him in coach it will cost you a time out" or should I assume he knows the rule and go directly to the table and tell the scorer to assess the team a time out? If the coach then jumps up and says "wait a minute I don't want a time out, I'll make a substitute," do you let him? If in any of these scenarios, the coach affirmatively says he wants a time out to keep the player in the game, does he get any additional time? Or do we start play right away? Dbyb
