Under NCAA rules, I believe that since Team B should have gotten the T as soon is it was obvious that the starting 5 wasn't going to be recorded any time soon (when the official inspected the book), Team A would shoot two free throws and the POI would be the opening jump ball.

You are correct. In NCAA there would still be a jumpball. In NFHS there would not be.
No one is obligated to go to the coach for this info. The team is responsible for supplying the scorer with their roster and starters prior to the ten minute mark. If those requirements are not met, a technical foul should be the result, but I'm sure that you realize far too many officials are reluctant to start games with a T.
