The R goes to B's bench, where the coach says his team should have been awarded a one-and-one on the previous foul by A against his team. The R consults the scorers, who say the previous foul was Team A's seventh. The R consults the U1, who the scorers say called the previous foul by Team A. U1 says he remembers the foul, but does not recall which Team B player was fouled. R and U1 then consult U2, who does not recall that a non-shooting foul occurred at all since Team A's last foul against B, which was in the act of shooting and resulted in two free throw attempts. The R also does not recall the intervening foul. The delay for the discussion has approached 10 minutes. The R rules that a correctable error for failure to award a merited free throw to Team B occurred, but that because the calling official does not remember which Team B player was fouled, the one and one cannot be attempted by any player from Team B, and so the game simply resumes from that point.

What should have been done?
That's all she wrote. If the R doesn't have exact knowledge, then he can't correct it. "Sorry, Coach B". -- and then don't forget to charge him with a 60-second time-out.
