A flagrant foul is judged by the severity of the act. If the intentional was not severe enough to be trying to injur the player, then it's still just an intentional foul. Now, if a team seems to be committing one intentional after another just for the sake of doing it (i.e., it won't change the outcome of the game), you might warn that team's coach about making a mockery of the game and to either knock it off or forfeit. That would be pretty extreme, though.
