I've never seen a ref get in trouble by referring to a player by number or to a coach as "coach." I have seen problems arise when refs cross that line of professionalism and give the appearance of being too chummy with a coach or player.

Personally, I'm just not comfortable calling a coach by his/her name during a game. We have all seen the opposing coach who is just looking for an excuse to cry favoritism. I have seen other officials call coaches or players by name and it hasn't generally led to a problem. However, I have also seen the exception where it led to problems.
