The 2nd two rules here seem to conflict as to when a player is required to take a time out, leave the field, etc.
LJ, you might want to use dashes for rules and dots for case plays like the books do, so as to not confuse anyone.

I believe the key issue is that the player must be replaced if the team has no timeouts or if they choose not to use one. That's really the purpose of 3-5-6.
