There is no requirement to present the ball. The ball can be in either hand or glove; visible or hidden.

No, not specifically pertaining to the pitcher. This may be considered obstruction of the batter (rule 8-1-1-d) or possibly unsporting conduct, or possibly nothing. If it was timed to when the pitch reaches the plate, I would not allow it; probably ruling unsporting conduct of a minor nature (warning to the offender, ejection if repeated).
