HOLY CHIT!!!! Over and OVer I kept saying to myself, and srcarfed spoonful after spoonful of humble pie trying to expain this to the guy.
I love youth games for the trouble that rears its head no matter how you handle things.
Curious as to how you guys would have explained that to the coach?

There is no satisfactory explanation, as far as the coach is concerned. You explain it once and eat one slice of humble pie, including one dignified apology, and one statement that you won't let any more mistakes sneak in. You let the coach complain for a few seconds to your face, then walk away. He gets a small amount of leeway after this, but don't let him eat you alive. Then you work your butt off the rest of the game. And be sure your partner knows that you don't appreciate being hung out to dry like this.
