We'd hold a short period of silence, and listen for the Holy Spirit to move us to consensus. As we listened, one or two might share with the whole group a leading from the spirit.

Then if that didn't work, we'd draw straws.

In my church, we'd have a congregational vote to establish a committee chairman (man or woman) who would then establish a committee. They would meet to determine the offended party, probably hold some interviews over pie and ice cream, and proceed to inform the pastor of their decision; at which point we would call a congregational meeting to vote on their recommendation.

All followed by a potluck dinner, of course.
