4) I think this is the same as 3. A player is oob until he has come in-bounds. To say that the foul occurred on the in-bounds side of the plane is false because the thrower is still oob. The foul is on an oob player and therefore intentional. Incidently, the thrower would have to reach pretty far through the boundary to have a foul occur on the in-bounds side of the boundary.
