The regular substitution rules apply -- a player who leaves the game can't reenter until time has run off the clock.

Yes, assuming the player is otherwise eligible (hasn't been DQ'd, hasn't left the game and time hasn't run off the clock).

Note that in FED, each throw can be taken by a different player. In NCAA, the same player must shoot both throws.
