Washington: Ratings go by the local association. In Seattle we fil out an evaluation on every official we work with. 10 classifications.
Preperation
Attitude
Rules Knowledge
Signals
Mechanics
Hustle
Poise
Alertness
Communication.

Each is given a rating of 0, 2, 4, 6, 8, or 10 (Don't ask!)
A rating of 0 or 2 requires a written explanation.

There is a coaches card that we hand to every HC before a game. It asks them to rate each member of the crew as:
Excellent
Very Good
Average
Poor
Unsatisfactory
There is a spot for comments about each official if desired.
These cards are mailed back to the association and are not seen by the official.
They are not used to rate officials, but are used to identify any problems.
The Washington Officials Association also has evaluators that travel the state and rate officials. These are retired officials who have been trained to view a game with an eye to what the crew is doing both as individuals and as a crew.
