Sorry Tornado, I thought it was clear. The entire plate and those "Bermuda Triangles" you refer to are in fair territory. Much like their namesake, they are kind of no man's land.
No, the batter would not be out if the ball is batted to the ground, hits there and comes back up at him. If he is completely within the box, those lines protect him during his at bat. In fact, on cruddy fields, the ball could hit IN FRONT OF the plate and spin back at him while he is still within the box and he's safe. That said, unless you have a great grounds crew and a perfect surface, you'll never be able to tell exactly where the ball hit.
