I've asked this question a time or two myself, and never gotten a clear answer. THere is no "official" signal. You stop the clock with a whistle and a violation signal, count the basket using the signal you'd use after a foul on a shot, and then look at the score table to be sure they understand, give the ball on the endline with running privileges. I think.

You nailed it!
The same is true for defensive BI. For offensive BI, you instead give the no basket (wave-off) signal and it is a designated-spot throw-in.
