1) Yes -- but I'm not sure I would have done so (depending on how egregious this was). You can give your partner the informal "rotation error" signal and if s/he trusts you s/he can make the call.
2) Eventually, this needs to be moved along. "mind changes" can be difficult for scorers.
3) Suggest to the captain that she needs a line-up check.
