The craziest thing I could find in a Twitter search was in the Miami-UCF game at 6:11 in the second half. The game was on CBS Sports Network. Not sure if that's the exact situation but it might be, I didn't look too hard.
In that situation, if the fouls happened in that order (PF, Class A TF, CDBTF, F2 TF), you administer all four sets of free throws in that order and the team offended by the F2 gets the ball.
