Delay sanction
 
USAV- the 3 minute interval between sets has expired, and team R has only 5 players on the court. Their captain points to the end of the bench, where a trainer is taping the leg of team R's sixth starting player and it appears that he is almost ready. Team R does not request a time out or substitution, how soon should the delay sanction be applied?

A DW is definitely warranted, but it's up to you to issue it whenever you want to issue it. If you're the type to issue a DW as soon as your stopwatch hits 0:00, then feel free to give it then (granted, that won't put you on anyone's good side).

However, giving them a couple more seconds will go a long way. If they're taking advantage of those seconds, then issue a DW if they opt not to take a time out or substitute the player while insisting on waiting for them to be ready before beginning play.

I wouldn't do it for at least 30 seconds, and I'd call that the injury evaluation time.
