Block

While blocking a hard-driven spike, the ball travels between the hands of the block and bounces off the head of the blocker. After coming off of the head, the ball was: (a) passed from forearms, (b) dug by setter, (c) hit off the shoulders. What's the call for (a), (b), and (c)?

There isn't one. All are legal.

Explain why you think any of the options are anything other than first contact (or second contact, depending on when the ball hit the blocker's head).

(EDIT: I'm trying to get you to talk through a better understanding of the rules, but to be honest, most of your posts come off like you're wanting answers to questions on a rules test without actually having to learn the rules and their applications. I hope that's not the case.)
