Injury time-out

In USAV, a team can be granted an injury time-out (lasting 3 minutes) when a player is injured and there are no other eligible players to replace him/her. This is what I learned this past weekend. (In related news, I also learned that I really need to get better at my rules knowledge.)

Is there any similar provision in the NCAA or FED rulesets? If so, how are they different from the USAV rule (if at all)?

Yes in FED -- if all other TOs have been taken and no legal OR EXCEPTIONAL subs are available. 11-4-2

Yes in NCAA -- but I *think* the injury TO is *before* the teams remaining TOs. 11.3.6.14

(can't help for USAV)

In USA, the OP is correct except for all time outs must also have been used.

In USAV, this is only if there are no legal or exceptional subs available. In essence, down to six eligible players.

I don't believe NFHS or NCAA have needed to drill down to this level as rosters are generally large enough that this shouldn't be an issue. Of course, there are small rosters now and then, but there's no provision like this in those rule sets.
