No such thing in NFHS, NCAA, ASA, PONY fast pitch; unless it's a league rule and then you have to ask the league.

USSSA alows the use of an EH and a DH in the same lineup.

This would result in a 10 person batting order, and 9 players on defense.

The DH bats only. The player she is batting for does not bat. (They are locked into the same position of the lineup card; therefor 11 players will be listed in the 10 person batting order.)

The EH can go on defense at any time. That is not a substitution.
