SP ASA - hit by pitch

Batter makes an attempt to get out of the way of a pitch, it hits him/her. Is this a hit batter, award first base?

Batter makes no attempt to get out of the way of a pitch and it hits him?

New ump and I do not have my book with me today.

In slow pitch its a dead ball. No award or anything like that.

Call dead ball, award ball, then check pulse to make sure batter is actually alive.
