Question

The Offensive Coach Subs for Catcher and that player reaches 1st. Upon end of play coach yells for a courtesy runner and another player comes on field takes her spot as catcher goes back into dugout. You look at your card and realize that the catcher they are running for has not yet played defense just subbing for the catcher that played defense in the last inning and will be playing there next inning. That player is not allowed to have a courtesy runner I know but now that the player is in the dugout and the other on 1st what do you do at that point.

Cool Coaches!

My answer would be preventive umpiring call TIME tell the Coach he cant do that and that the SUB needed to go back to 1st before play resumes

I agree with Oppool, except that you should not need to call TIME since you hould have already dne that when the coach asked to put in a courtesy runner.

Now what do you do when the savvy defensive coach waits until the pitcher has thrown a pitch to the batter before bringing it to your attention?

Yes, But that player was already in the dugout and once you leave the field of play you cannot return.

As SAMNVA stated since a pitch has not been thrown the situation is still correctable. Tell the defensive coach it was a mistake which is corrected by putting the SUB back on 1st base and play on

On SAM's question after a pitch then you would have to tell the offensive coach if brought to your attention by the defensive coach that the CR that came in for the SUB is now a SUB that is in the game and if SUB1 still is eligable they may re-enter in the same spot if coach wishes.

This notion only applies during a live ball.

Doesn't make any difference. The ball was dead, therefore you just place the proper player on 1B.
