Fouls by the offense that occurred behind the basic spot. The basic spot can vary depending on the type of play. For example a run that ends behind the LOS has the previous spot as the basic spot. But if the run ends beyond the LOS the basic spot is the end of the run. If the offense has a holding penalty behind the basic it is enforced from the spot of the foul. An exception to this though is if the foul occurs against an opponent behind the line of scrimmage. Fouls against the ball (i.e. intentional grounding and illegal kicking) would be enforced from the spot.
I use this as just one example. Penalty enforcement isn't always a simple statement. There are often many factors. Some specific fouls that are often "spot fouls": Kick catch interference Illegal kicking/batting Defensive pass interference (less than 15 yards beyond LOS) Intentional grounding/illegal forward pass |

Yes and no. It's a live ball foul enforced like any other live ball foul using three and one. In most cases the spot of the foul will be behind the end of the run, but there could be instances where the foul is beyond the LTG or it is behind the LOS. UNS fouls by the defense also would not be enforced at the spot.
