A few months ago I posted a thread about forms for basketball official critiquing. Now I'm looking for some for football officials. We're trying to implement something this year, and I was wondering what other people are using.

Any links or suggestions appreciated.

NASO has an excellent publication re evaluating officiating performance. I recommend it.
