Rich NFHS Rules: The clock starts on the RFP. The clock always starts on the snap when B gets a new series, but only after a legal kick when A gets a new series (or another major clock stopper).
Theisey Since I time NCAA games, on a possession change: A to B and back to A, the play clock will set to 40 seconds and the game clock will start on the Referee's wind. The exception is as TXMike points out when under 2 minutes and team-A runner went OOB.

Now if I can just remember this until the next time it happens.
