Evaluations/Assessments/etc.

I am interested in knowing how evaluations are done in various associations. Is the coaches ratings the only evaluation? Is there an evaluation system in use that does not involve the coaches or utiltizes the coaches plus others? Does the evaluation system utilize video? Does the evaluation system utilize an on-site evaluator and, if so, what is the background of the evaluator, ex-official, college official, etc.?
