Is there any Substitution infraction if A11 breaks the huddle and thinks they have 12 players and runs off the field? Thanks

No. In fact if a 12th man breaks from the huddle you don't necessarily have a foul. It depends if he leaves "immediately" which by most interpretations is in the 3-5 second timeframe.
