Some final comments. I believe the situation we are discussing is covered by penalty item 4: "If there is a violation first by the free-thrower's opponent followed by the free thrower or a teammate:"
I believe this statement covers our situation. The confusion is in 4.b: "If the second violation is by the free thrower or a teammate behind the free-throw line extended and/or the three-point line, both violations are penalized, as in penalty item (3)."
The wording in 4.b leaves out the possibility that the second violation could be by the free-thrower's teammate in a marked lane space. BUT if both violations are by players in marked lane spaces then refer to 4.a.
To clear up this confusion I would suggest that 4.b should have "behind the free-throw line extended and/or three-point line" eliminated. Then 4.b would read: "If the second violation is by the free thrower or a teammate, both violations are penalized, as in penalty item (3)." The possibility of both violations are by players in marked-lane spaces is already covered by item 4.a.
