Substitutions
 
A6 enters the game during a dead ball and replaces A5. Can A7 replace A6 before the clock has started or in other words during the same dead ball?

Yes. The only exception is once a player has been substituted OUT of the game he cannot re-enter until AFTER the game clock has been started legally.

Yes, there are no playing time requirements.
