Player steps out of the lane just as the second whistle is blown. Official probably glanced up and decides he was out of the lane when he chooses to recognize the request. Correct position for throw in if that's the case.

Nope, that's still wrong. The diagram extends well outside the lane. Still should have been unbounded on the end line. They just got it wrong.

It was pretty close to the point where the "pyramid" meets the free throw line. I looked again and it looks like he awarded based on the ballhandlers position after he walks out after the whistle. Definitely sloppy/lazy.

Possibly a good point to emphasize teamwork by a crew...non-calling official might have helped by observing ball location after partner hit his whistle.
