Has anyone ever even heard of this rule being applied? :D

If you mean rule 10-3-6d, then the answer is yes. (Rules use hyphens, cases use dots.)

Happens sometimes in MS games. Warn the player, warn the coach, try loudly to talk her out of it, then T if it continues.

Saw it in an NBA game last year. It was a Celtics game and (surprise) Garnett was involved. The defender actually had his hand ON Garnett's face before the official called it. Was kind of funny, but the defender forced the call.

You sure it wasn't a 6th grade "competitive" game? Maybe it was 5th grade varsity.

As far as the OP goes, I've never had to use the rule. Warned once and that's the closet I've come.

It was not competitive, and even by Billy's liberal "Webster Standard," it wasn't "varsity." There wasn't even a national anthem played. :D
