Free throw situation

A1 is free throw shooter and ball is at her disposal;

1) A1 steps on the line while holding ball, steps off free throw line, begins throwing motion and releases the ball
2) A1 steps on free throw line after the ball is released, and before ball hits rim/backboard
3) A1 releases the ball, losses balances, and steps into lane before ball hits rim/backboard
4) A1 bounces the ball on the free throw line, with both feet off the line, and then shoots the ball.

Which is a violation?

Which is a violation?

By rule, 1,2,3.
